Student Employee Supervisor Forms

  • Payroll Authorization Form - Complete and Submit when Student is Hired 
  • Position Description Form - Complete for Approval of New SE Positions
  • Job Posting Form - Submit to have SE position posted on Handshake
  • Webtime Approver Change Form - for Student Employee Approvers
  • Budget Number Modification Form - When Org or Fund # Changes on a Position
  • Termination Form - Submit when Student Employee Leaves a Position
    (not graduates or end of semester),  
    Exit Check List Form - Supervisor Reference

  • Summer Exception Request Form - Departments who have a critical need for student employees to work more than the 25 hour per week limit (all positions combined) during Summer Semester, submit the Summer Exception Request Form
  • Performance Evaluation Supervisor Forms and Information
  • Accommodations - Supervisors/Employers, if you have a student employee that may need accommodations, have the student employee review the Americans with Disabilities Act (ADA) policy and procedure and then have them complete the form to request accommodations for their work place needs.
