- Payroll Authorization Form - Multiple Student Entries - Complete & Submit for Multiple Students Hired in the Same Position.
- Position Description Form - Complete for Approval of New SE Positions
- Job Posting Form - Submit to have SE position posted on Handshake
- Webtime Approver Change Form - for Student Employee Approvers
- Budget Number Modification Form - When Org or Fund # Changes on a Position
- Performance Evaluation Form (PDF Fillable)
- Use the performance evaluation form above to complete for each of your Student Employees. (at least Annually during Spring Semester and/or prior to a student leaving a position.)
- Termination Form - Submit when Student Employee Leaves a Position

- Summer Exception Request Form - Complete and Submit if your department has a critical need for student employees to work more than the 25 hour per week (all positions combined) limit during Summer Semester.
