Fieldwork Site Requirements

Contract

The contract serves as a legal document between the fieldwork site and Xavier University’s occupational therapy program. The contract states the rights, fieldwork requirements, and obligations of the academic program, fieldwork site, and students. A contract is required for all Level I and Level II fieldwork placements. Xavier University has a standard contract, which you can use.

AOTA Data Form

The purpose of the AOTA Data Form is to facilitate communication between occupational therapy (OT) and occupational therapy assistant (OTA) academic programs, OT/ OTA students, and fieldwork educators. Fieldwork Educators and Academic Fieldwork Coordinators (AFWC) jointly complete the Fieldwork Data Form to describe the fieldwork setting where students may have placements. This is extremely helpful for students to gain information about a fieldwork site prior to beginning the fieldwork placement.

Xavier University’s occupational therapy program annually updates the fieldwork site’s information in an effort to collect data that help us meet ACOTE standards for fieldwork. We recognize that completing the AOTA Data Form is a big time commitment, so students can assist the fieldwork educator in collecting this data and providing you with a completed electronic copy that you may update as the information changes over time.

If your site already has a completed AOTA Data form that you provide to universities, please send a copy of that document directly to the Academic Fieldwork Coordinator or to the student so it can be submitted to the Academic Fieldwork Coordinator.

Level I Fieldwork Objectives

Level I Fieldwork Objectives allow academic and fieldwork educators to agree on established fieldwork objectives prior to the start of the fieldwork experience. All fieldwork experiences include a psychosocial objective (ACOTE standard C.1.3, 2018).

Level II Fieldwork Objectives

Level II Fieldwork Objectives allow academic and fieldwork educators to agree on established fieldwork objectives prior to the start of the fieldwork experience. All fieldwork experiences include a psychosocial objective (ACOTE standard C.1.3, 2018).
