Terms of Agreement

By submitting the request for materials in accessible media you agree to and understand the following terms:

• I understand that it is my responsibility to inform Accessibility and Disability Resources of any changes made to my schedule i.e. adding, dropping or section changes.
• I understand that it is my responsibility to have access to a purchased/rented copy of the material for the duration of my access to the accessible media copy. If I lose access to the purchased/rented copy, I will delete or return the accessible media copy. I will submit proof of access as required.
• I understand that after requesting materials in a accessible format, materials may take time to receive (up to approximately 6 weeks). I am responsible for notifying Accessibility and Disability Resources if I am not receiving my materials in time for me to prepare for class.
• I understand it is my responsibility to monitor my Xavier email address for questions about my request and access to my materials.
• I will contact Accessibility and Disability Resources if I am having trouble accessing my accessible materials.
• I agree not to copy or reproduce the accessible media or share them with anyone else.
• I understand that any reproduction or distribution of my accessible material will be considered a copyright infringement and disciplinary action through Student Contact or other legal action could be considered.
