Xavier University encourages the Xavier community to engage in healthy behavior and proactive health management Employees hired prior to July 1, 2020 can qualify for the calendar year 2021 medical wellbeing rate by completing by September 14, 2020 1. an annual preventative physical with their Primary Care Physician, 2. a biometric screening, and 3. an online health risk assessment.
The medical wellbeing rate is a $50 per month ($600 per year) decrease from the standard rate.
All three calendar year 2021 Medical Wellbeing Rate Requirements must be completed and documentation must be submitted between September 15, 2019 September 14, 2020.
If you do not have a Primary Care Physician and you are currently enrolled in Xavier's medical insurance, Anthem's Find a Provider tool can assist in finding an in-network provider.
