Medical Insurance Request

Per the Travel, Meals and Entertainment Expense Policy, employees traveling internationally on behalf of or utilizing funds administered by the University are required to enroll in the University’s international insurance plan through Cultural Insurance Services International (CSI). Upon completion of the form, the Center for International Education will enroll you in the insurance plan. Please submit information no later than two weeks before departure date. The cost for up to one month of coverage is $42.75.

Faculty and staff leading and participating on approved curricular or co-curricular programs do not need to fill out this form to be enrolled in CISI insurance.
