Caracole (HIV Case Manager)

HIV Case Manager: This position provides intervention and case management services for HIV+ individuals and their family members. Case Managers must adhere to standards of care as set by Caracole, provide HIV-related teaching, facilitate access to case management, medical care, medication access, oral health care, psychosocial support and referrals for a broad range of services.
