ABSN Student ID Upload Form

                                                  

The Mobile ID, Xavier One Pass, is the official form of identification for the Cincinnati Xavier University community. For the ABSN Columbus and Cleveland sites, the ID Card is the form of Identification necessary due to the hardware on location.  These two locations will not get the One Pass, they will receive an ID Card at Orientation.  For all to obtain a Xavier ID, it is required that you submit a high-quality photo of yourself to be used for your One Pass or ID card prior to your arrival on campus.  The ID photo will also be used on the student clinical badge which will be received at Orientation.

Please be sure the photo you send follows the points outlined below. We reserve the right to not accept a photo that does not adhere to these points.

The photo MUST be...

• at least a 800 x 600 pixels in dimension (it can be larger, just not smaller).
• a high-quality, clear, color photo.
• No filters; original photos only.
• Use a plain, lighter colored background.
• a front view of your whole face, with you looking straight at the camera.
• taken within the last 12 months.
• taken in normal clothing without a hat or dark glasses.
• The preferred file format: .JPG
