Student Concern Form

WHAT HAPPENS ONCE A CONCERN REPORT IS SUBMITTED?

Where Does the Referral Go?

Once a concern report is submitted, it is reviewed by Care and Support Services and members of the X-ACT Team to determine the most appropriate next steps. Depending on the situation, information may also be shared with other Xavier professionals who can assist in providing support.

In most cases, the person who submitted the referral will be contacted by an assigned staff member within one to two business days to acknowledge the report and discuss next steps.

Will Someone Meet with the Student?

Based on the information provided, a staff member may reach out directly to the student or collaborate with other campus partners to assess the situation and coordinate support.

When meetings occur, they are intended to be supportive, not disciplinary. Our goal is to help students navigate challenges and connect with resources that promote their success and well-being.

Not every referral results in direct outreach to the student. In some situations, our team may provide consultation and guidance to the reporting individual and recommend appropriate follow-up actions or resources.

What Updates Are Provided:

When possible and appropriate, we will provide updates to the individual who submitted the referral. However, the information we can share is often limited by federal privacy laws, including FERPA.
