Exhibit Student Submission Permission Form

I give my permission for the Xavier University Library to include my work done in the following course(s) in the library’s institutional repository, Exhibit. I understand that works in Exhibit are publicly available on the Internet and searchable in Google, library databases, and other search engines.

For example, Chemistry, Education, English, etc.

If you are a student please provide course name.

Include last name, first name, and degree.

Have your received authorization to submit your work from your Faculty member?
