In order to issue a Certificate of Eligibility (Form I-20 or DS-2019), please complete the following form. If you wish you may also submit a paper version of this form.

• Incomplete forms will not be processed.
• You can upload documents on this site (we'll automatically receive them along with your form)
• The Official Bank Statement must be original (no copies, faxes, internet print-outs, etc.)

• SEVIS Transfer Form (Must be completed by the student and signed by the International Office at the student's current school)
• Sponsor Statement Form (Must eb notarized if the sponsor lives in the U.S. or if the sponsor resides in the U.S.)
