Root Cause Analysis

If you are trying to solve problems but they seem to keep reoccurring in different disguises then you are probably working on systems rather than the root cause.  Often we state the problem in a symptom or solutions format and find we are not making progress at all.  For example, we may say, “our problem is we don’t have enough training.”  We presuppose that training is the answer!

Root Cause Analysis is a systematic approach that establishes a concise, valid problem statement, then investigates the data or situation that is causing the problem.  A pareto ( 80/20 ) analysis uncovers which cause is affecting most of what is observed.  The solution, based on the pareto chart, will make the biggest impact at reducing the impact of the problem.

This approach maximizes the resources used which makes it an ideal economic choice for a problem solving model.
