CREDIT EVALUATION INSTRUCTIONS
If you would like to know how credit from another institution will transfer into a degree program at Xavier University, you can request a preliminary credit evaluation, by returning the completed information card below to the office of admission, along with your transcript of courses and grades. Please note that a transcript, computer printed grade report or paper based grade report are all accepted. Please reference the list of undergraduate programs available for more information. You can return the form and transcripts by fax to 513-745-4319.
