Checklist

1. After departmental approval, complete the Studio Abroad Application.
2. Complete a specific program application. This process will also require an initial $50 application fee, payable via CASHNet, our secure payment processor.  Scroll down to "Study Abroad Application Free" and click "View Details".
3. An official transcript for non-Xavier students. A temporary PDF copy can be submitted through the application above. An official copy needs to be mailed to Xavier by February 7:

4. A $500 deposit, checks payable to "Xavier University". Mail to:

5. The completion of:
   • AOR 115: Assumption of Risk Form. This needs to be notarized.
   • MED 202: Emergency Medical Care Authorization and Health History Form. In addition, a copy of your insurance card needs to be attached and a physician's signature is needed.
   • PA 303: Student Agreement Form.
6. A copy of the front page of your passport - the photo page. If you do not already have a passport please apply for one as soon as possible. If you already have a passport, please make sure it expires after November 2013.
