Registration Form

These files are in Portable Document Format (PDF).   You must have the Adobe Acrobat PDF Reader to view them.  To download a free copy of Adobe Acrobat Reader, click on the Acrobat icon.

If you have Adobe Acrobat Reader but have never used it before, you will have to launch the application first.   Your browser will ask you to "pick app."  Select "browse" then go to your C: drive, in the Acrobat3 folder.  Select the "Reader" folder then open the "AcroRd32.exe" file.  Click OK.  After you have launched Acrobat Reader the first time, it will automatically be opened any following time that you try to view a PDF file.

Graduation Application and Walker Application are updatable. Before printing, please type your information in the desired fields. It is also possible to reset the form by clicking on the "Office Use Only" box in the lower left corner of the form.

Paper copies of the forms are available in the Registrar's Office in Alter Hall.

To request a paper version of the catalog, schedule of classes, registration materials (Fall & Spring: graduate students only, Summer: all students), address/name biographic information form, or graduation application, be sent to you in the mail click here.
