MRI Application Form

All applicants must meet the University's admission requirements and be accepted by the Center for Adults and Part Time Students as an undergraduate student.  Applicants can apply on-line using Xavier's website at www.xavier.edu/caps.  You will also need to send your MRI application form and a copy of your ARRT certification card to:

 If you are a graduate from Xavier University, you only need to submit the MRI application to Donna Endicott.
