National WebCheck Fingerprinting

Fingerprint services are available at the Xavier University Police Department for any member of the Xavier University Community.

Where: Flynn Hall

When:  Monday 1pm  to 4pm

            Tuesday 9am to 12pm and 2pm to 5pm

            Wednesday 9am to 12pm

            Friday 1pm to 5pm

Most background investigations require electronic submission of fingerprints.  Some require fingerprint cards; Xavier University Police does not offer these types of fingerprinting  services.  You can contact the Hamilton County Sheriff’s Office at for assistance.

The cost of electronic fingerprinting is $60.00 for State of Ohio and FBI checks.  Individual being fingerprinted will be billed through the Bursar's Office for the background check.  We do not accept cash or credit cards.

You should allow seven to ten working days, but it may take up to thirty days, for the results of the check to reach their destination. If you need to know if the background check was received, contact the department it was sent to. The Xavier Police Department cannot check on the status of the background check or if it has arrived. If the the department that the background check was mailed to did not receive it and it has been longer that 30 days, contact the Ohio Attorney General's Office at 877-224-0043.

 

What do I need?   Please read the appropriate paragraph below for instructions. 
School of Nursing
  • If you are student in the School of Nursing complete the WebCheck form online, print it and bring the completed form, including the consent pages, and your government-issued photo ID (i.e. driver's license, passport).   Because many government-issued identification documents do not include the Social Security Number, you must know the number in order to be fingerprinted. 
    • The BCI&I Background Check Reason will be 4723 09 Nurses (RNs, LPNs, dialysis techs. students entering nursing education or dialysis)
    • The FBI Background Check Reason will be 4723 09  Nurses (RNs, LPNs, dialysis techs. students entering nursing education or dialysis)
    • The direct copy can be sent to the Ohio Board of Nursing if you need one. You are responsible for determining if a direct copy is needed.
    • The mail to copy should be sent to the School of Nursing
School of Education
  • If you are student in the School of Education complete the WebCheck form online, print it and bring the completed form, including the consent pages, and your government-issued photo ID (i.e. driver's license, passport).   Because many government-issued identification documents do not include the Social Security Number, you must know the number in order to be fingerprinted.
    • The BCI&I Background Check Reason will be NOORC Other: Care, Custody, and Control of Children.
    • The FBI Background Check Reason will be NCPAIVA Volunteer Children's Act.
    • The direct copy can be sent to the Ohio Department of Education if you need one.  You are responsible for determining if a direct copy is needed.
    • The mail to copy should be sent to the appropriate education program you are in.
  • If you need to apply for or renew your teaching license, use the following codes:
    • The BCI&I Background Check Reason will be 3319 39 B3 School Employees- teachers only
    • The FBI Background Check Reason will be 3319 39 Public School District or Chartered Nonpublic School Employees and School Bus Drivers
    • The direct copy can be sent to the Ohio Department of Education. You are responsible for determining if a direct copy is needed.
    • The mail to copy can be sent to the appropriate education program you are in or school district or location of your choice.  Remember only one mail to copy is provided.
All other programs and organizations:
  • Complete the WebCheck form online, print it and bring the completed form, including the consent pages, and your government-issued photo ID (i.e. driver's license, passport).   Because many government-issued identification documents do not include the Social Security Number, you must know the number in order to be fingerprinted. 
    • The BCI&I Background Check Reason will be NOORC Other: Care, Custody, and Control of Children.
    • The FBI Background Check Reason will be NCPAIVA Volunteer Children's Act.
    • The direct copy can be sent if you need one.  You are responsible for determining if a direct copy is needed.
    • The mail to copy should be sent to the appropriate program you are in.
  • If you need to apply for or renew a license or need a background check for a job outside Xavier:
    • Select the correct BCI&I Background Check Reason.
    • Select the correct FBI Background Check Reason.
    • The direct copy can be sent to the list state agencies.  You are responsible for determining if a direct copy is needed.
    • The mail to copy can be sent to the location you would like.  Remember only one mail to copy is provided.
    • You are responsible for obtaining the correct information to complete the form.

If you are under 18 years of age you must have you legal guardian complete the Juvenile Waiver and bring it with you when you are fingerprinted.
