ACCEPTANCE

Acceptance process for Guatemala 2010:

1. Eligibility to join Guatemala 2010 will be determined by:
• Contents of X. U. Study Abroad Application for Admission
• Contents of two Personal Reference Form
• Results of interview conducted with the GA 2010 team.
  
  
2. Preference will be given to:
   • Persons speaking Spanish
   • Persons with pediatric experience
   • Persons determined suitable for this experience (maturity level, readiness for experience, ability to work well in a group).
     
     
3. You will be notified via email of your status (accepted or declined) on or around Dec 4, 2010.
   
   
4. Upon acceptance you must complete the following forms and return them to the Department of Occupational Therapy by January 15, 2010:
• Study Abroad Program Emergency Medical Care Authorization (Note: this form requires proof of insurance with medical, evacuation, reunion, and repatriation of remains coverage - Xavier’s student health insurance does include this)
• Health History Form
• Health Screening Examination (Note: this form requires signature by a physician or other health care provider – the health care provider can be obtained through the McGrath Health & Counseling Center).
  
  
5. Due on the day of the packing party (as notary will be present to sign and seal):
   • Assumption of Risk and Release for Guatemala 2010 - Students (this form must be notarized with a signature and seal)
   • Academic Service Learning Agreement - Students (this form must be notarized with a signature and seal)
     
     
6. Give deposit money (non-refundable $100.00) to Department of Occupational Therapy by Jan 19; amount will be applied to bursar bill.
