Xavier faculty, administrators, staff and students can now nominate students of African descent for the 34th Annual Antonio Johnson Scholarship. Please download the nomination form and return to ML 2311 by November 20, 2009 at 5:00pm for full consideration.
Sustainable Efforts
Multicultural Affairs is committed to supporting sustainable efforts throughout Student Life and Leadership and across campus. In an effort to communicate and keep the environment clean, please click on the forms you need to submit, fill them out online and then bring them to a staff member.
Calendar Event Requests (click to download form)
If you'd like your event to be featured on the Multicultural Affairs Calendar, please submit the following form at least 30 days in advance. Please note, it must have your Advisor's signature to be complete.
Room Reservation Requests (click to download form)
If you'd like to meet with a small or large group in our space--Gallagher Student Center, Room 280--please fill out the following form. It is first come, first serve, so plan ahead. We offer a Library space, conference table, social space and audio/video equipment for your meeting needs. If you'd like to use our audio/video equipment for a presentation (PowerPoint or DVD) you must make an appointment with a staff member before your event.
Weekend Room Reservations (click to download form)
Because we do not have professional staff available on the weekends, you are able to request a room key by filling out this form. You must be a current Xavier student in order to have room key checked out to you.
