To take an exam/test in our facilities please complete and submit the form below, or you may visit our office to sign up. Please allow us 2 working days prior to your examination for processing. The fields marked with an asterisk(*) are required.

Finals Testing: You must sign up by the Friday 2 weeks before finals. Students who do not show up within 5 minutes of their scheduled final exam will forfeit their preferred seating.
