Sign up for a test

To take an exam/quiz in our facilities, you must follow the LAC test taking procedures. Please sign up at least 3 working days prior to your exam by completing and submitting the form below, or you may visit our office to sign up. The fields marked with an asterisk(*) are required.

Finals Testing: You must sign up by the Friday 2 weeks before finals.

Upon submitting this form, you will receive an e-mail confirmation. Keep this confirmation until after you have taken your test. If you do not receive a confirmation, your request has not been submitted and you must go to the LAC to sign up for your exam.
