The office of Risk Management & Insurance will arrange for issuance of a Certificate of Insurance as proof of Xavier’s coverage(s). If you are required to show evidence of the university’s insurance coverage and need a certificate, please fill out this form.
If available, please provide a copy of the insurance clause or paragraph in the agreement to clarify what is being requested. You may fax to the office of Risk Management & Insurance at (513) 745-2092.
With this information, it is usually possible to obtain a certificate within 48 hours. If a fax number is provided, the certificate will be faxed as well as mailed by the university’s insurance agent.
