Insuring a Vehicle

This form contains required information for scheduling a vehicle for insurance coverage with the university's insurance carrier.

The vehicle will be scheduled within 24 hours of submittal of this form and will remain on the university's insurance policy until you provide notice to remove it from coverage. Please see the lower portion of the form to remove a vehicle.
