Plan Changes 

(Effective January 1, 2015)

♦National Point-of-Service (NPOS)* deductible increases to $500/$1,000 (from $300/$600 in 2014)        

♦High Deductible Health Plan (HDHP) deductible and out-of-pocket maximum increases to $2,600/$5,200 (This increase—up from $2,500/$5,000—is based on IRS inflation adjustments)

♦Tobacco cessation drugs will be covered at 100% under the drug plan

♦Preventive breast cancer drugs and lung cancer screening now covered at 100%

♦Residential treatment centers are now covered as any other illness

♦Tobacco surcharge will begin July  1, 2015 ($30 per tobacco user per month)

♦Dependents will be covered until the end of their  birth month they reach age 26.  Dependent coverage does not extend to age 28

Employees who do not make online enrollment elections during Open Enrollment will be defaulted into their 2014 benefits (with exception of FSA and HSA elections). FSA and HSA elections will not roll over to 2015.
