Immunizations are an important tool for protecting yourself and the campus community from illness. Full time undergraduate students are required to have documentation of two measles, mumps and rubella vaccinations (MMR). Additionally, students 21 and younger are required to have documentation of one meningitis vaccine received at age 16 or older. Incoming first year students must complete the Medical History Card- Immunization Record.
*Please note that according to Ohio Revised Code 3701.133, Students living in a campus residence must complete Section VII (disclosure of Meningococcol and Hepatitis B vaccination status) of the Medical History Card-Immunization Record.
Tuberculosis screening is required for international students coming from high incidence countries.
Students are to input mandatory immunizations and upload Immunization records into Xavier's online student health system. Links to complete this process are forthcoming.
