On-Campus Employment Forms

Students residing outside of Indiana/Kentucky/Ohio are to use the Ohio State tax form using their permanent home address.  Student residing in West Virginia/Kentucky/Indiana may elect to use the Reciprocity State tax form.

• Direct Deposit  - Students who do not elect to have their check directly deposited into a bank may pick up their paycheck in Schmidt Hall Monday - Friday between 10:00 a.m. and 2:00 p.m. only.
